This window can be found by clicking on the star button at the far right of the URL box, then clicking the "Edit" button. I have attached a screen capture of the problem. I am using Kubuntu 11.04 and it seems like the button is part of Kubuntu's theme for Chromium, so perhaps there is a corrupted graphic somewhere in Kubuntu's theme.
