Results in screen corruption in small or large areas of the form which make
it unreadable or unusable.
Notified them of trouble and they said " update to Ubuntu version of Adobe
flash player"

This same problem occured last year at this time with PBS survey form
Not sure if this is Nvidia problem or Adobe.

Just did a splash of updates that were probably relased yesterday.
using Ubuntu 13.10 on Dell Inspiron 9300.
Problem is reproduceable and occurred again after new adobe flash player
install
