Use only install/update tasks for installing menu items

We should add all menu items via update tasks instead of bootstrap files
because otherwise it is really hard to figure out why this and that menu item
appears.
We probably want to expand the Update Task/Delta API for menu items a bit
(remove, move, more convenient constructors etc.).
Also see MAGNOLIA-1410 which describes changes made for 3.5.
