### What problem does this PR solve? Issue Number: resolve Problem Summary: Manual reference releases can be skipped when callbacks throw, leaving `hide()` or destruction blocked indefinitely. Additionally, `count_exposed()` reads map sizes without synchronization while concurrent exposure or hiding modifies them. ### What is changed and the side effects? Changed: Add `ScopedExposedRef` to release acquired references automatically on scope exit, including early returns and exceptions.
