The questions are good, the survey is short and I think it's best to keep it that way. You could dig deeper into the 'Have you migrated apps from Cordova to alternative solutions? If yes what did you use?' - question. For example 'Why did you migrate your app to an alternative solution? ' (Too much native requirements?, Decision from the company?, Lack of interest or trus in mobile hybrid development? ... ) - This question can also be answered in the 'What are your pain points with Cordova ?' - question.
