Ken, The end point in the ctakes 4.0 for CVD is not a valid end point, I wrote to UMLS to confirm and this is what they have replied to me
That endpoint (documented here: https://uts.nlm.nih.gov/help/license/validateumlsuserhelp.html is not meant for end users, so it will not work with your license code and username.
