+1 for the default publication. Some of our customers want a more flexible notification GUI, e.g. entering e-mail addresses on the publish confirmation screen manually. But this could be implemented in a custom publication.
to get rid of the other issue, why not have separate workflow items "publish" and "publish subtree"? this would also get us around the UI ambiguity that i mentioned in another thread ("workflow buglet in 1.4?"):
