There is a weird effect with line annotations, that can be seen with page 9 of
the example PDF. The rectangles of several annotations have an empty surface,
because the two y values are identical. When I bring Adobe Reader to create the
missing AP streams (by changing an annotation text and saving the file), the
new PDF file has annotations with different rectangle values than before, i.e.
Adobe adjusts the rectangles so that the annotation display fits.
