Updated in v5:
- rect adjustment even if rect not empty, see PLPDF.com-MarkupAnnotations.pdf
(but improves other files too)
- rect adjustment for strikeout
- adjusted position in strikeout to be closer to Adobe
- support correct strikeout positions for diagonal annotations
- fill-only support (no stroke when 0 width or no color) support for circle and
rectangle
- squiggly annotations now painted (but only like underline for now)
