It worked in the file I uploaded. Maybe you didn't make all the changes I
mentioned.
I can't help you without the result PDFs. See all my comments - each time I was
able to tell the exact mistakes by inspecting the PDFs with PDFDebugger. So
without the PDFs I can't tell. If your PDF is confidential, then sign a non
confidential PDF. If your image is confidential, then use another one.
