You could collect all the rectangles (cyan color in the example, plus type 3 
best guess), make an assumption which ones are near enough from others to be a 
"block". Then extract these blocks with ExtractTextByArea for each, 
alternatively set "beads" (see PDF specification) and call the normal 
ExtractText only once. Of course there may be special cases, e.g when a block 
is not rectangular.
