In commit 47108ec i've removed all application handling from the plugin and
also cleaned up the lifecycle participant. It can now work on any number of
features contained in a directory.
In addition I correct the lifecycle mapping, and added the attach features mojo
as a default
The substitution is now done in the lifecycle participant, avoiding the
generate resources mojo completely
The aggregate features mojo needs a clean up as well
