We oftentimes get software grants and CCLAs submitted with different names for
who signed the document versus who submitted it as the point of contact. Based
on the existing forms, this ends up sending an email with the To generated with
the signer's name but the contact person's email address. This results in
confusing emails.
To address this problem, I suggest adding an addition field for the contact
name to be used in the generated email To header and "Dear X" line. The signer
name should remain in use for the commit message and commit line in the email.
