As SME Server ships with a binary of qmail, it would be a violation
of Dan Bernstein's distribution conditions if it was shipped with any
modifications at all (even mods suggested by the author himself).

Gordon has suggested you read the previous thread on how to achieve this
without modifying qmail at all. I concur.

The FAQ entry you quoted tells you exactly how to do this.

If you plan on distributing a binary with these modifications, be aware
that you will be in breach of the conditions referenced above. (If you
are doing this for yourself then feel free).

Also, if you do make this change, be aware that you must make certain
that the delivery instructions in the .qmail-log file DO NOT result
in the message being re-injected into the queue. Re-injection will
lead to recursive deliveries. The safest way is to ensure the message
is delivered to either a mailstore (such as a mbox or Maildir/) or is
processed by a program that does not inject the message into the queue.
