Hi Rich,

Perc 5/e is not using aac driver but mega_sas driver. I am not sure whether 
the issue impacts mega_sas driver too.

I have been running NexentaStor 3.1.2 based on Illumos SunOS 11 on IBM x3650
with Perc e/e controller with DELL MD1000 direct attached storage. I had no 
issues with this setup.

When I tried to upgrade to NexentaStor 4.0.2 I hit the issue when OS can see
through Perc 5/e RAID controller only a single logical volume from the 
storage. Basically all the devices are detected but disabled by Fault 
Management Facility (check the boot log). Because disk devices for other 
logical volumes are retired, I cannot access data on them but megacli 
utility can see all the logical volumes configured. As soon I boot from 3.1.
2, I have no issues to see all the logical volumes again.

I tried to reset/repair indicated faults, but when I reboot they come back 
right away. So far I have no solution or work-around for this problem. I 
prvided URLs to relevant log files and the output of fmdump and fmadm 
utilities. I hope that somebody can point me into right direction to solve 
the issue.
