An in-the-wild example I could point to would be something along the lines of what Nintendo does with their Club Nintendo signup form.
https://club.nintendo.com/registration.do Like Robert said, inline validation a must, keep it as simple as possible, and don't break the user's expectations. In contrast to the above advice, Nintendo breaks expectations with the final section giving the user 13 inputs as opposed to between 1 and 4.
