(This is more Canadian centric posting).

voip.ms is a discount provider/reseller (there's a couple of others like thespout.ca). It's fine for personal use - business use, not so much. Also, they bill in USD$ if that's an issue.

My ranting on dealings with them - random horrid call quality (due to software based transcoders from what I've been told), fairly large amount of outages and worst: bad/false sip registrations so calls won't complete because they decided to re-route your did to a different server causing "this number is not in service" messages until you re-establish the registration. Notifications of issues generally a couple of hours after they happen. These kinds of issues are what kills them for business use that needs their phones constantly 8hx5d or more.

Overall, I'd offer thinktel.ca as one of the better quality canadian providers for businesses, but not always the cheapest (ie: on/off net disparity sometimes, but that's a per-customer-usage-destination thing), but they've been straight up in dealing with issues (especially true with doing ports) on the phone, and a fairly powerful portal. Only 1 outage (~10min) in a couple of years that I can recall.

Just a quick overall observation, but out of my dealings with voip providers and Canadian phone issues - if they know what the CRTC is and what that entails (ie: more paperwork usually), then you're generally better off.

I've had great luck the last two years with VoIP.ms[1]. I believe they
satisfy all of your requirements.

They *do* let you select your own DIDs, though whether or not you'll be
able to find an free range of DIDs that match your internal extensions
is up for question.
