We use the Servertech failsafe pdu*, but as a 3U pdu, it doesn't
fit your specific requirements of a single-unit device.  (But it does
do 120v and modular power cords)

A heads up though: Something that we only picked up on at the last
minute - for some ATS units out there, your input power feeds must be
phase synced, or you could pass a nice voltage spike down the line
which the power feed is switched. This can be interesting to deal with
if you use multiple UPSes.
