headers in latex to appear only once

Hi all

Would anyone here happen to know how one might create a header in  
latex that only appears on the first page?  I am writing an essay in  
MLA format where I am supposed to have my name, my teacher's name,  
the subject and the date all in the left top corner of the first  
page.  I can't figure out how to make it so that the header won't  
appear on every page.
