I see what you mean but I don't know if you can highjack multiple
sources and mix them together in real time using Audio Highjack. A
rather crude way (but it would work) would be to use an omni directional
mic (if possible) and get the output from VO via external speakers
attached to your PC. This way you would get the output from VO and your
voice into Audio Hijack via your mic. If its a reasonable mic the
quality should be OK. Pay attention and experiment with mic positioning
so you get a reasonable balance between your voice level and the output
from your speakers.

I use an really crappy quality web cam mic during user tests at work and
can record the output from JAWS and what people are saying quite well,
as long as the PC speakers are up to a reasonable level so the mic can
pick up the screen reader output also.

Another alternative would be to use a sequencer like audacity
http://audacity.sourceforge.net/ instead of Audio Hijack. Add a new
track (Project> New Track>) and this enables you to use both VO and to
speak to your Mac, even without using the mic, via the built in mic that
Intel Macs have. The quality is not high fidelity but it works well.

Would be better with a good mic though.
