I have Vitelity for business, I do recommend them. Do keep in mind,
they have some strange issue when passing rfc2833, you may need to use
inband DTMF.

VOIP Your Life does BYOD as well. So does Vitelity (obviously, and
asterisk counts as a device); Voicepulse has a BYOD plan too, if
I'm not very much mistaken.
