Ok thanks, I've changed forms.py like this:

class BookForm(forms.Form):
book_item = forms.ModelMultipleChoiceField
(queryset=Book.objects.all(),
widget=forms.CheckboxSelectMultiple)
But the problem how to access at the fields of book in the template"
still remain
