I found that I had a few changes in a local repo from last year for readability and making the points about information disclosure more explicit. While reviewing those, I noticed that references to numbered sections in the other two documents were no longer correct. So I pulled those things together and it's been merged into the GitHub repo for the failure reporting document. It was done as a pull request, so it can easily be reversed. Or ignored.
