If you've just installed TortoiseCVS, you should bear that in mind as 
well.  It has some serious flaws in its implementation which I'm currently 
discussing with Francis.  They can certainly cause Explorer 
crashes.   However, if you're seeing a crash on doing a !, it sounds more 
likely to be DQSDTools.dll.

Basically, you need to get us the address of the crash.  I have arranged 
DQSDTools.DLL to be at a (probably) unique address, so it should be easy to 
find the culprit.

If you can have drwtsn32 (it comes with Win2K) running during a crash, that 
will create a good log.   I'm not sure what the default behaviour during a 
crash is - maybe drwtsn will get started anyway.
