Thanks. You sent me in the right direction, and in case it comes up again,
what happened as follows: I use NoScript, a Firefox security add-on, which
blocks Java on sites which you haven't allowed it for. I added the file
address as it appeared in my Firefox address bar
(file:///C:/DOCUME~1/JOHNWR~1/LOCALS~1/Temp/DQSDLaunch.html) to the
"Whitelist" in NoScript, and everything works. Thanks again.

I'm speculating, but one reason for this may be that Javascript is disabled
for your browser. There's a bit of javascript in the temporary file you're
seeing which should submit the form, and if that wasn't firing for whatever
reason, this would probably be a symptom.
Check under Tools > Options > Content to see if Javascript might be
disabled.
That's all I can think of currently.
