I have seen a similar problem in quake 1 and 2. Whenever I'm diving or 
wearing a bio-suite or quad damage the entire image gets slight color 
change, which I think is just a transparent polyogon. However, in some 
levels of quake 1 and in all I've seen of the quake2 demo sometimes I 
get completely blindfolded. This seems somehow dependent on my viewing 
direction and position in the scene. Maybe this is easier to track down 
than the phantom tux.
