My current settings in BIOS:

  Aperture: 128MB (previously was 64 -- wouldn't work at this setting)
  AGP Capability: 4X
  AGP Drive Strength: Auto
  AGP Performance Control: Enabled
  AGP FastWrite Control: Enabled
  Video Memory Cache Mode: UC

I'll disable Performance Control and Fast Write.
