with regards to Step 2, how did you expose the metadata in the form?
With form I assume you mean [dspace-source]/config/input-forms.xml?
There are several input types to assign to a field among others date,
see dc.date.issued as input form field:
<field>
<dc-schema>dc</dc-schema>
<dc-element>date</dc-element>
<dc-qualifier>issued</dc-qualifier>
<repeatable>false</repeatable>
<label>Date of Issue</label>
<input-type>date</input-type>
<hint>Please ...</hint>
<required>You must enter at least the year.</required>
</field>
So setting your dc.description.embargo (might be better not to use dc
for this but rather use an internal administrative field) as input type
date in the input-forms should display it as such, assuming that you did
build, update and deploy your installation afterwards. There is no step
3 so maybe this is missing.
Furthermore the embargo will only work on newly ingested items, i.e.
during the submission process. Adding the configured embargo field as a
metadata to an item in archive, will have no effect.
