No, you don´t need. Some repositories, as

Handles are useful to offer persistent URLs (PURLs) in the sense given
by IETF rfc1737 <http://www.ietf.org/rfc/rfc1737.txt>
The main consideration arises because you are offering to the public a
"not-so-persistent" identifier. If in the future, your organization
changes the name and therefore the domain, the external references
(for example, the ones included in CVs..) to the documents will change.
And that can be a terrible mess. But if the repository is made of
photos and videos, the impact "could be" insignificant. As noted, if
you operate a repository with researcher´s output, that is always a
problem...
What if I go live in our production environment without registering an
official handle and without running a handle server ?
Almost nothing happens. If you later reconsider you decission, you can
install one, pay to the CNRI, renumber all the "internal" references, by
example changing all the 123456789 prefixes to the one assigned by the
CNRI with a Dspace command, etc...
The only problem are the external references, that will not resolve to
the new renumbered objects...
Do I need a handle ?
If you mean a handle server, the answer is no.
Do I need to run a handle server myself ?
Can't I just use the default handles generated by the "default"
installation of dspace ?

You can also adjust the rules of URL assignation with some parameters in
Dspace config
