Yes, this occasionally happened to me, both on 1.8.x and 3.0. I think it happened to me only once on 3.0, where OAI no longer consumes DB connections, so I recommend upgrading as a workaround. There is no solution yet because we still have little information about the problem - it's hard to pin down the conditions reproducing it reliably.
