That's what I figured you wanted, but hoped it was for something less all
encompassing. See my interlineations:

A cordless phone for your modem

Allows you to use your modem without a physical connection to a phone line

via the RJ-11 jack

a modem (built-in or external) and nothing else (all is included with the
thing)

I bought mine for $50 a year ago, I've seen them for as much as $100 and
as little as $40

I got mine from PC Connection, although I think you can buy them directly
from IBM

AFAIK

a modem (internal or external), an active phone connection and an
electrical outlet to supply power to the base unit

It's about the size of a small brick divided into two parts. It weighs
about 1/2 pound. The receiver end is rechargeable or you can run it off
power as well. The receiver end can be attached directly to your laptop
lid (and is removable) or you can carry it separately. It operates at
900MHz and has about a 150 foot range. I've never had a problem using it
either faxing or surfing the web from the middle of my backyard.
