I've experienced this once or twice myself.
Never been able to intentionally duplicate it and have always assumed I
goofed up a shut down procedure.
I have a hunch, purely a hunch, that it may have been related to shutting
down DXB with a telnet or vhf packet connection still active.
But I can't certify to that.

Anyway, Tony's fix of opening the TaskManager and closing DXB that way
always corrected the problem instantly.
