For non-members and members defined externally, there is no distinction 
between "Highlight in file" and "Highlight usage in file". However, for 
members defined locally this distinction is important and I believe 
"Highlight usage in file"* *should be the default. I discovered recently 
that Ctrl-Shift-F7 also works on members that are defined outside the 
current class such as invoking on the append method of 
stringBuffer.append("xyz"). Originally I only expected that it would 
work for members defined in the current class.

In support of this request, at times I have even found myself pressing 
Ctrl-Shift-F7 on text that isn't a member just expecting (hoping?) this 
to work but then have to remind myself that so far it only works for 
members.
