building an X12 variant using an Insurance subcommittee guide there are
usage codes of "R" and "S"...Has anyone seen this before and know what
they [stand] for?"

Dear Steve:

You're undoubtedly referring to the usage of segments, loops and
elements within a Health Care (HIPAA) implementation guide.  The "R" and
"S" stand for "required" and "situational," respectively. The front
matter of the HIPAA guides explain the usage:  Required means you must
use the segment, loop or element; and Situational means the usage
depends on....well, uh.... the situation.
