I rely on my MicroHam Microkeyer to key the rig  and amp.  You can set the 
delay in the MicroKeyer.

You can also  insert a half character space at the beginning of your CW 
macros so there is a  slight delay between going into transmit mode and 
actually 
transmitting.   In the N1MM contest software the appropriate character to 
insert 
is the  ~
