If it was a size problem, the sender should have received a bounce 
message telling them it was too large.

If the sender received no notice, and you received no notice, and the 
email with attachment simply vanished into the void... AND they have 
tried sending it again with the same result... I'd say it probably is 
being flagged as spam for some reason. It may have something to do with 
the email body text content. Possibly particular words being used are 
setting off a high enough spam result.

They can try once more, this time using a simple, non advertising word 
laden body text (something like "we have no idea why this didn't go thru, 
but we will try it once more").
