Do you per chance have a "clean" OS X machine with a "clean" OS 9 install for Classic you can try it on and see if the problems continue?
If not, have you previously tried making a new user, and trying it under the new user to see if the problem continues?
This is assuming that you have verified that it is in fact NOT a stray temp file being left behind, which is what you seem to indicate.
