Hi Rich,

like to comment on your table in regards to Europe:

There is no such definition 'Yellow with green stripe' or 'Green with
yellow stripe' known to me to be in existence in CENELEC.

The correct definition for colour coding for Protective Earth per IEV
(International Electrotechnical Vocabulary) is 'Green and yellow', where
either colour shall cover the surface per length unit between 40 and 60
percent.

Within a building and/or an equipment there is no distinction made
concerning the colour of PE except where blank metal constitutes a part
of the PE system.

In cordage you can find black/blue/green&yellow or brown/blue/gn&yel. 
 
Within a building L can be black or brown. But there are some exeptions
as well... (can also be blue in some cases). Better test before
touching!
