Pierre, According to the text of IEC 60601-1, the test should be performed with AC voltage, and does not give the option of performing the test with DC voltage, as is the case in other standards.
The standard allows some components to be disconnected during the test, but primary-earth capacitors are not mentioned. Nevertheless, I believe that it may be possible to use engineering judgment and test using DC voltage, giving the justification in the test report. I give as an example the data sheet at: http://www.ipdpower.com/pdfs/SRP-25.pdf This power supply range has UL approval to UL2601-1 and TUV approval to EN 60601-1. In the "Application Information" in the data sheet, the following appears: "This power supply has been safety approved and final tested using a DC dielectric strength test. Please consult factory before performing AC dielectric strength test." I do not know what justification UL and TUV used, but they seem to have found a way to get around the problem.
