Richard, For our 2-42GHz wireless radios, I use OSN on line to notify most EU member countries. There is no cost involved for this service.
https://webgate.ec.europa.eu For Poland, Italy and a couple of other countries I mail the notification letter using US postal service. DoC needs to be issued by manufacturer and not test lab as it is the manufacturer who is always responsible for the product not test house.
