Nothing is immediately looking wrong.

First I would suggest putting a halmeter on
motion.spindle-index-enable to see if it is being set back to zero at
the start of the threading pass.
(Machine->Halmeter in the Axis GUI menu)

You have a net for spindle-at-speed and a "near" that looks like it
might be intended to drive it, but the rest of the linking is missing.
Just try commenting out the "net spindle-at-speed    =>
motion.spindle-at-speed" line. It is just possible that linking the
HAL pin to a signal that is not set anywhere else is setting
motion.spindle-at-speed to zero, which will disable fed moves.
