I'm running IHSSE on my Exchange 2K3 box. Reasonably happy, catches probably 93-95%, requires some time to setup and maintain, as you add users and global whitelist/blacklists to improve your kill ratio. Installation is easy and support is good. Sunbelt maintains a discussion list for users which helps. I've not used the freeware solutions.
