"Once per address per out-of-office period" applies only to the Out of Office reply text. All rules fire for each message. Period. You can take that to the bank. Some people make the mistake of thinking they need to augment their Out of Office message with a reply rule and end up annoying everyone.

The risk of a loop from an Out of Office without a rule is small because the Out of Office message is sent only once to each sender.
