Are these users in a different OU container that isn't selected to pull updates 
in your ADC?

I have a single site, mixed-mode.  I still have 2 Exchange 5.5 SP4 mailbox 
servers with about 400 users on them, and 1 Exchange 2003 mailbox server with 
about 2100 users on it.  (I also have a couple of front-ends.)  My forest is 
Windows 2003 Native, with about 28 domains (consolidation is my next project).  
All the Exchange servers are in one domain.

Here's the issue.  Some of the new users I've set up in the last few weeks (I 
added the ADC and set up the recipient update services over New Year's) have 
complained of not receiving external email.  They receive in-district email 
just fine.  They show up in the GAL on the 2003 server (and the Outlook GAL for 
users whose mailboxes are on the 2003 server).  They do not show up in the GAL 
on the 5.5 server.  This has only happened with a couple of them; most are 
working fine.  I don't get it.  Why would random accounts not replicate to the 
5.5 servers???

Oddly enough, the ones that this has happened with are all in the same domain 
as the Exchange servers.  They are mailbox-enabled users, not hidden from the 
address book, and SMTP and X.400 addresses were properly generated.  They can 
SEND outside the district, but even a reply to one of those sends generates an 
immediate NDR.  If I sit and watch on our GFI mailfilter, I can see the message 
come in and be rejected by the 5.5 server that's still running the IMS.  
Unknown recipient.  Well, duh, since they don't exist to the 5.5 server.

I did rebuilds to all the ADC's and RUS's in the hope that would do something; 
I also ran DS/IS consistency adjuster on the 5.5 servers.  It's been a couple 
of days, and there's still a couple users not showing up on the 5.5 
infrastructure.  WTF???

I even tried deleting their mailboxes, removing their mail attributes, purging 
them from Exchange, and re-creating the mailboxes from scratch.  Still no luck.

This one really has me scratching my head.  What am I missing, and why is it 
only happening to a handful of accounts?
