We use two St. Bernard's ePrism1000 appliances in a cluster mode. It can
also handle AV.
It works pretty ok. Once in a while we get spam, but it's very minimal.
Users can also have some control. Suspect spam emails end up in their
Junk Folder.

The reasons for the selection about 2 years ago were:

*       A self-contained unit with web interface that can be directly
attached to the network
*       Dependant only on an external MX record for operability
*       Built-in redundancy either through hardware or software
*       No impact on the ability to send/receive email in the event of
failure
*       Can utilize real time black hole lists and a distributed
checksum clearing house
*       Can utilize Bayesian filtering, malformed message protection,
heuristic message scanning, configurable white lists and black lists,
and reverse DNS lookup
*       Can provide another layer of defense against email borne
viruses, with ability to schedule updates for virus definitions via the
web
*       Contains user controlled lists and quarantines, which can be
configured and managed by the end user, without needing third party
client side software.

. Can be used to generate usage reports for incoming emails, and Spam
related messages, as well as for denied incoming messages, including the
policy that prohibited the inbound message.

. Comparable or equal cost to other solutions such as: Iron Mail, McAfee
SpamKiller, Tumbleweed, & Barracuda.

Overall, I think you need to make a decision based on first identifying
what's important to your company (system requirements), risks involved,
and how much it'll cost to implement and manage the solution (software,
hardware, time, and human resources).
