If the SMTP address is a proxy address it won't show up unless someone opens her properties and looks a the e-mail addresses tab. I suspect far fewer people do that than the number of outside-the-company correspondents who might use the old SMTP address, but that's for you and her to decide.

If nobody will be replying to old messages sent when the mailbox's legacyExchangeDN or Exchange 5.5 DN was one of those two addresses, then you're safe to remove them. If you have no compelling reason to delete them, then it won't hurt anything to leave them alone except that it increases the size of your Active Directory by a few bytes. The 2nd one has her maiden last name. She wants the 1st one removed. Will removing that X500 address and leaving the other one, create a problem? Is there such a thing as primary & secondary(ies) X500 addresses?
