It sounds, from the comment about the old name popping up whenever someone sends her a message, that you're actually talking about the Exchange directory name popping up as part of the autocomplete offered up by Outlook 2003. Deleting the X500 address won't stop that from happening. AFAIK nothing will, short of deleting and recreating the mailbox.
