John et al, I had the exact same setup and scenario ... I found MS technet answers that addressed this problem and I went through every step (and more) to no avail ....
the only thing that worked was to uninstall OWA ... remove the exchng folder and reinstall ... I even had it working on a box (same setup as you) for 2 weeks and then it crashed back to the 3 logon , blank page scenario ... I have since mirrored the drive (it is working now) ... I will remove the mirror and let it run until it breaks again (it has already broke 3 times) and then try to trace down the problem ... of course I have a backup OWA in the shadows ready and willing to go ...
