It was a 'Send as'. 
Here is the scenario: 
UserA has permissions to log on to USERB's mailbox. UserA wants to send an
email using "USERB"'s address to about 500 recipients (SEND AS). UserA sends
an email to 500 receipients using the BCC field with USERB's email address
in the FROM: field - included in this list of 500 receipients are 2 internal
distribution lists. The email went through and is sitting in UserA's SENT
ITEMS folder but the email didn't come through to any of the members on the
internal distribution list. On opening up this message, the BCC field is not
displayed and on clicking on VIEW, the BCC FIELD entry is greyed out and it
isn't checked either. So I can't verify the list of receipients to confirm
whether it might have been user error or a genuine problem with the mail
server.
