it is all users - domain admin, exchange service account, regular users,

It redirects to my back-end server and says error 404 file not found.  The
redirect URL is "http://ohca/MBX/USA/user_name"; which has me confused
since there is no such path.

User logged on to the network from win2k pro and server machines.

Not sure what you mean by this.  The event logs still say unknown username
or password.

Have tried all of the following: //computer_name/exchange,
www.domain_name/exchange, IP address/exchange, and have also added
/user_name to the end of all of those with no luck.
